Completing Enrollment at HPHS

If you are visiting this page, it is because you are interested in having your family join our Harlem Prep High School community for the 2020-2021 academic year! We are excited about the opportunity to get to know you and support you through the enrollment process!

Upon accepting a seat at HPHS, follow these next steps in the given order to ensure having a successful enrollment process:

STEP 1: Review our Course Catalog
Compile documentation PRIOR to your enrollment meeting.

  1. Proof of Age:
    • Birth Certificate AND copy of passport
  2. Front facing photo of scholar
  3. TWO Proofs of address dated within the last 3 months (must include the name of the adult enrolling the scholar)
    • State ID
    • Utility bill
    • Cable Bill
    • Government Letter
    • Rent Receipt 
    • Lease (with a valid date)
    • Tax bill/ letter
    • Voter Registration
  4. Physical Form (dated within the last 12 months)
  5. Immunization records (Up to date, if not, including doctor’s appointment letter to catch up)
  6. Medical Administration form (This is for prescribed medication, must be filled out by a doctor)
  7. Most recent report card (Final report card must be turned in, in order to finalize enrollment for proper grade placement)
  8. IEP/ 504 (If applicable)
  9. School Record Release Form
Annual Documents 
You will have to sign paper copies of these documents at the enrollment meeting.

  1. Housing Questionnaire
  2. Media Release
  3. Field Trip Waiver
  4. Meal Application
  5. DPPS Family Handbook
  6. Physical form (if your scholar wants to get working papers or play sports, physical must be up to date)
  7. Medical Administration Form (if applicable, see previous slide)

Step 3:
Set up an enrollment meeting with Ms. Rodriguez. Enrollment meetings typically last 45 minutes to an hour depending on questions asked.
